Responsible for assisting Social Workers/Counselors in providing assessment, prevention, and crisis intervention through home-based and family focused support services. Adhere to fidelity of program model, principles and practices. Maintain an assigned caseload. Assist with initial intake with client and family/guardian. Maintain timely documentation in agency database system (ie. assessments, service plans, safety plans, case notes, discharge plans) and other required records in accordance with agency, state accreditation, and other requirements, policies, and procedures. Ability to work flexible hours, including some evenings, required. Must have strong written and oral communication skills and a strong working knowledge of computers and applications. Travel primarily within the county assigned, but some statewide travel is required as needed. Must have valid driver’s license, which meets the requirements of the agency’s insurance carrier and reliable transportation.
Bachelor’s Degree in Social Work, Counseling or related field required.
Full-time (37.5 hrs/week) with benefits.
