Citizens Care & Rehabilitation Center is a long term care & sub acute rehab facility in Harford County Maryland. We are in search of a Full Time MDS Coordinator with Resident Assessment Coordinator Certification (RAC-CT).
Current Maryland (or compact state) Registered Nurse license with at least 2 years clinical experience.
This position is responsible for coordinating and facilitating MDS assessments interfacing with the care plan process and communicating with multiple internal disciplines
Ensures accurate, timely and optimal completion of MDS assessments
Informs disciplines of assessments due, completes Nursing assessments, monitors daily for completions, coordinates forms for signatures, transmits and reconciles
Ensures all disciplines contribute their section of assessment in a timely and compliant manner. Acts as resource for MDS for all disciplines
Responsible for reviewing resident and guest charts daily or as applicable for supportive documentation of skilled services including observation, order changes, ADL documentation, therapy notes, labs, etc.
Schedules MDS utilizing PCC software
Coordinates and manages PPS process by monitoring acuity levels, overseeing and analyzing both MA and MC RUG classifications; communicates with Therapy and Physician, acts as Medical Assistance and Medicare coverage resource and collaborates with team regarding ongoing daily needs
Acts as liaison with therapy, nursing, families, residents and guests to carry out care plan and achieve resident restorative goals
Coordinates with therapy in setting ARD dates in relation to diagnosis, skilled services, goals, ADL documentation
Chairs daily PPS meetings
Interfaces with billing department; facilitates required documentation including Medical Eligibility Review form (3871)
Reviews quarterly CMI reports verifying accuracy.
Participates in the MA review and validation process quarterly, annually and as needed
Participates in the development, implementation, communication and evaluation of resident care plans. Ensures staff is implementing care plans, delivering care appropriately and recognizing changes in residents’ status
Reviews facility Quality Indicators (QI) and Quality Measures (QM) intervenes as necessary
Identifies problems and reports and makes recommendations for addressing problems to the Director of Nursing
Prepares written and oral reports for the Director of Nursing
Supports and demonstrates practices consistent with the mission, philosophy, standards, policies and procedures in the nursing department
Attends continuing education programs and reads current nursing literature to maintain competency and improve knowledge and skills
Participates in the development and presentation of in-service education programs in support of staff development. Identifies educational needs of employees and makes recommendations to the Director of Nursing
Provides direct care as needed. Obtains assistance for clinical emergencies and assists as necessary, including CPR
Ensures resident and guests rights are protected and ensures they are free from abuse and neglect
Adheres to infection control practices
Attends In-Service training to maintain and improve competencies
Other duties as assigned
Current Maryland or compact state RN/LPN license. Two years clinical experience in a long term care facility. MDS experience and Resident Assessment Coordinator Certification (RAC-CT), preferred

Practices professional nursing codes of ethics
Requires excellent verbal and written communications skills, and a professional demeanor
Projects good judgment and performs all duties satisfactorily while prioritizing projects as assigned
Exhibits excellent customer service skills and attention to detail
Requires knowledge of current State and Federal regulations pertaining to MDS
Possesses good working knowledge of Microsoft Office Suites
Physical and Environmental Requirements:
Must be able to stand, sit, stoop, push, pull, balance, reach, grasp, kneel, climb and lift up to 50 pounds. Exposure to blood borne pathogens
