Provides psychosocial assessment, screening, counseling, and crisis intervention services on consultation basis to patients and families, utilizing a variety of treatment modalities including individual, family, group and specialized counseling. Works collaboratively with the multidisciplinary team to determine the need for services; to effect positive patient and family outcomes; and, to assist in securing community resources to promote continuum of care.
EDUCATION: Master's degree in Social Work.
EXPERIENCE: 1 year social work or related experience; experience in an area of specialty (i.e. dialysis, rehabilitation, home care, oncology, or hospice)
LICENSE/CERT/REG: Social Worker license in the State of Maryland.
SKILLS: Psychosocial assessment and advocacy skills. Excellent verbal and written communication skills. Working knowledge of various computer software applications.
